Comparison table midwife assistance contract - as at 18.4.2025

Yesterday, the new midwife assistance contract was finally published on the GKV website.(Download here). We have looked through the contract for you and created a practical Excel list that you can use to make your own calculations.

The new midwife assistance contract, which comes into force on 1.11.2025, is currently the subject of much discussion. Instead of billing with flat rates per item, from 1.11.2025 there will essentially be a switch to time-based billing. One item will be charged for every completed 5 minutes. Travel expenses and other items have also been increased.

Comparative calculation of a midwife with 6 outreach appointments in one day

We have now compared a real day of a midwife we know as an example. The midwife made 6 appointments and 4 consultations on this day and drove a total of 78 km. So far, she has generated income of €339.85, from 1.5. it will be €354.42 (+4.29%).

If the midwife works exactly as before from 1.11., she will earn €409.92 (this corresponds to an increase of 17.09% on the current rate)!

However, the new contract means that longer visits will be better remunerated. If the midwife were to make full use of all quotas and, for example, needed 90 minutes for each postnatal visit, then an increase of up to almost 54% would be possible. The disadvantage of this solution, however, is that the midwife would then need 8 hours and 15 minutes of pure care time instead of the current 4 hours and 10 minutes. In reality, the increase will be somewhere between these two values.
